Terms and conditions for Healthcare Professionals Membership

FIT4MOM LAKE COUNTY MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM LAKE COUNTY and is a month to month agreement and except as provided in the initial 10-day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited classes provided by Fit4Mom Lake County, excluding programs.

AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM Lake County to initiate a charge to the account entered into Pike 13 for my dues. This authorization is to remain effective until I have canceled my membership in writing through the FIT4MOM Lake County administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Lake County to fulfill my payment commitment and I am obligated to pay. Dues will be drawn on the first of every month (unless otherwise specified) from the account on file on the day your membership begins monthly. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be canceled.

CANCELLATION POLICY

To cancel this membership, the member must request a cancellation via email with the subject line, MEMBERSHIP CANCELLATION/CHANGE to zerinahutchinson@fit4mom.com. Cancellations will not be accepted unless requested via email and will be confirmed in writing to the member once they have been processed. Your cancellation request has not been processed until you receive a cancellation confirmation email from us. All cancellations require a 30 day notice and if not requested before 30 days, you will be charged for the upcoming month. You must submit your cancellation request via email. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Lake County franchise and is non-refundable after the first 10 days. Instructors are not authorized to cancel memberships. Our franchise can also terminate memberships at any time if terms and conditions are not adhered to.

HOLD POLICY

Holds must be requested via email with the subject line, membership hold. Once a request is submitted, a confirmation will be sent to you via email. Hold requests must be received at least 14 days before your hold begins. There is a $25 hold fee that will be assessed for any non-medical hold. This fee is non-refundable and non-transferrable. Holds will be automatically placed for a period of 2 months but can be removed at any time. At the end of the hold period, membership billing will resume automatically. It is your responsibility to keep track of your hold status. If you need to extend your hold, this process must be completed again at least 48 hours before your hold is set to expire. Hold fees will be assessed for any additional holds put on your account and memberships can only be placed on hold twice in one calendar year (Jan-Dec). Any billing on your account after your hold period cannot be refunded or transferred. Instructors are not authorized to place a hold on memberships

INITIAL 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after your initial payment transaction. To cancel this agreement written notice via email must be received by FIT4MOM Lake County within the initial ten days. You are responsible for paying for the classes already attended at the drop-in rate.

Liability

Fit4Mom Lake County, Fit4Mom HQ as well as any of the locations and facilities we frequent are not liable for any damage, injury or death that occurs during class time, arrival or departure. The franchise and other entities are also not responsible for any occurrences that happen to children who are in their strollers or at moms side during class time.

Kids Rules

All children attending Fit4Mom Lake County classes or pre planned events/playgroups are under the supervision of their parent/adult only. Our instructors are there to teach fitness classes, we are not babysitters. All children under the age of 4 must be SEATED & BUCKLED in a stroller or wagon at all times during class. Older kids are expected to sit on a mat next to mom during class/ walk with mom while traveling during class. Please use your best judgment to determine if your child is mature enough to stay on your mat. Moms are welcome to do trial runs with their big kids and this can be discussed with Zerina via email. All children outside of the stroller are also required to have a Big Kid Waiver filled out that can be requested at zerinahutchinson@fit4mom.com with the subject line, Big Kid Waiver. Fit4Mom Lake County instructors and staff can/may ask you to leave at any point if you are not following our kids rules as well as your own behavior (negative, hurtful, dangerous). Every mom will have two warnings to buckle their children/seat their big kids before being asked to leave class for the day. This is for the safety of all parties involved, mainly your children.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with the proper advance written notice.
